Almost every patient who walks into a dermatology clinic for pigmentation has already tried something first. Lemon juice, a home peel, a friend's leftover cream, a serum an influencer swore by. By the time they book a consultation, they've usually spent months, sometimes years, on remedies that did little beyond irritating the skin further. Pigmentation and melasma are among the most common reasons people search for a dermatologist in Santacruz or look up pigmentation treatment in Mumbai, and also among the most misunderstood. Mumbai's climate doesn't help: high UV exposure through most of the year, humidity, pollution, and long hours indoors under blue-light-emitting screens all feed into the same problem. 'Pigmentation' is a broad term for any patch of skin that has more melanin than the skin around it. Melasma is a specific, chronic form of pigmentation that tends to show up as tan, brown, or greyish-brown patches, usually across the cheeks, forehead, chin, and upper lip. It is far more common in women, particularly during pregnancy or while on birth control, which is why it's sometimes called the "mask of pregnancy". TheAmerican Academy of Dermatology notes that women with medium to dark skin tones are most likely to develop it, which makes it a routine, not rare, concern in a city like Mumbai. A large Indian multicentric study of 331 melasma patients across four regions found the female-to-male ratio was roughly 4:1, with a mean patient age of 37.2 years. Just over 30% of patients had a family history of melasma, and centrofacial (42%) and malar (39%) were the two most common patterns of distribution. Only 35% of patients in the study were using sunscreen at all, and just 10% were using one with SPF above 50 (Sarkar et al., Dermatology and Therapy, via PMC). A separate case-control study conducted at a private dermatology clinic in Mumbai found melasma was significantly associated with being 35 or older, being married, and having a family history of the condition, with family history nearly five times more common among melasma patients than among controls (study via PMC). In other words, genetics and hormones set the stage, and sun exposure decides how the story plays out. This is probably the oldest home remedy in the book, and it's also one of the most damaging. Lemon juice is highly acidic, with a pH around 2, against skin's natural pH of roughly 4.5 to 5.5. It also contains psoralens, compounds that make skin far more reactive to sunlight. Dermatologists describe the resulting reaction as phytophotodermatitis: a chemical burn that often leaves behind pigmentation darker than what was there to begin with (IOL Lifestyle, citing dermatologist Dr Angelique Wagemaker). Scrubbing has a similar problem. Melasma and post-inflammatory pigmentation form deep in the skin, well below the layer that a scrub or a loofah can reach. Aggressive exfoliation doesn't touch the pigment; it just irritates the surface, which for pigmentation-prone skin often triggers more pigment production as an inflammatory response. Gentle exfoliation has a place in a skincare routine. Scrubbing pigmentation away is not one of its jobs. Sun exposure is a major trigger, but it's rarely the only one. Hormonal shifts from pregnancy or oral contraceptives, certain medications, thyroid conditions, and even visible light from screens and indoor lighting all play a role. This matters clinically because treating melasma as a purely cosmetic, sun-related issue means missing the actual drivers in a given patient. It's also why melasma tends to recur even in people who are otherwise careful, since a trigger like hormonal change can't always be avoided the way sun can. Price is not a proxy for the right treatment. A ₹15,000 serum with the wrong active ingredient for a patient's specific type of pigmentation will do less than a correctly prescribed, far cheaper topical. What actually determines outcome is an accurate diagnosis of the type and depth of pigmentation (something a Wood's lamp or dermatoscope exam can establish), followed by a treatment plan matched to it. This is also why over-the-counter routines built on guesswork so often stall out after a few months. This is the myth that causes the most frustration, mainly because it isn't entirely false; it's incomplete. Treatment can fade existing pigmentation substantially, but melasma in particular is a chronic, relapsing condition. Creams and procedures can fade the discolouration, but they don't cure melasma outright, and it's common for it to resurface with unprotected sun exposure, especially in summer (AAD). The realistic goal is long-term control, not a one-time fix, which is precisely why maintenance and sun protection matter as much as the initial treatment. Hydroquinone has picked up a bad reputation online, largely from misuse of unregulated, high-strength formulations bought without supervision. Used correctly and under a dermatologist's guidance, it remains one of the most established treatments for melasma. The AAD lists it, alongside tretinoin with a mild corticosteroid, and the "triple combination cream" (tretinoin, a corticosteroid, and hydroquinone together), as core prescription options (AAD). The risk isn't the ingredient; it's using it without a diagnosis, without the right concentration, or for longer than advised. Sunlight increases pigment production and can undo months of treatment progress. But not all sunscreen protects equally against melasma specifically. Regular UV-filter sunscreen doesn't block visible light, and visible light, particularly high-energy blue light, is now understood to be a real trigger for pigment darkening, especially in medium to deep skin tones. Tinted sunscreens containing iron oxide address this gap. A randomized trial found that a sunscreen combining UV and visible-light protection reduced Melasma Area and Severity Index (MASI) scores by 75%, compared to 60% for a UV-only sunscreen (study cited in Practical Dermatology). A separate randomized, investigator-blinded study conducted in France over a full summer found that melasma patients using a visible-light-protective tinted sunscreen saw significantly less relapse than those using an untinted sunscreen of the same SPF (study via PMC). For patients in Mumbai, where sun and screen exposure are both high year-round, this is one of the highest-impact, lowest-risk changes available, and it's also the one most people skip because plain sunscreen "should be enough." Tranexamic acid, taken orally or applied topically, works by interrupting the pathway that triggers excess melanin production. It has moved from a niche add-on to a mainstream option because the numbers back it up. In a retrospective analysis, 89.7% of patients on oral tranexamic acid documented improvement, typically appearing after around two months of treatment (Dermatology Times). A 2025 randomized trial comparing oral versus topical tranexamic acid over 12 weeks found MASI score reductions of 58.86% and 50.88% respectively, both statistically significant (Heydari et al., Journal of Cosmetic Dermatology). A meta-analysis pooling 11 studies and 667 participants found tranexamic acid reduced MASI scores by an average of 1.60 points on its own, with oral dosing showing the strongest effect (Acta Dermato-Venereologica). It isn't right for everyone. Dermatologists screen for clotting risk before prescribing it, which is one of several reasons this is a treatment to start under supervision rather than self-medicate. Chemical peels, microneedling, and laser or light-based treatments can meaningfully speed up results when layered onto sun protection and topical therapy, rather than used as a stand-alone fix. The AAD notes that laser and light treatments show improvement specifically in patients who are already using topical medication and consistent sun protection (AAD). Where these discussions fall short is depth: a forum post can tell you an ingredient worked for someone else, but not why it worked, what concentration was used, how it interacted with their specific pigmentation pattern, or what checks were needed before starting it (tranexamic acid and clotting history, for instance). That gap is exactly what a proper dermatologist consultation is for. Not every dark patch is melasma, and not every melasma case responds to the same protocol.
